You should have received your Blue Cross Verification of Coverage Form

Blue Cross / Blue Shield will be mailing 2015 Verification of Coverage form to all members in May. Even if you don't have other coverage, you must complete the form and return  it by July 15, 2015 to ensure continuous coverage.  If you have not received your VOC form by June 15, we suggest you call the 800 number on the back of your Blue Cross card. (800 422-9146)
